This version of the State EMS Forms was updated on this site on 08/12/2010, and revised by the state on 06/08/2006.

The Georgia Office of EMS/Trauma has implemented a new, web-based, statewide data system called GEMSIS. This system gives Georgia's 272 licensed ambulance services the flexibility to collect their own data and then upload it to electronic standard.

In March of 2006 GEMSIS began the process of collecting EMS data electronically.  In 2008, approximately 635,000 Patient Care Records were entered by the State’s 288 EMS agencies.   State regulations require EMS agencies to collect and report call data to the DHR.   State EMS service providers currently utilize a variety of methods to import their patient care reports (PCR) into the GEMSIS system.  These methods include direct entry of data into a web-based collection system, conversion of data entered into software developed by other vendors for individual agencies, and through the scanning of ‘bubble’ reports that are manually completed by the EMS providers. The system was developed for the State of Georgia by ImageTrend, Inc.
