This version of the State EMS Forms was posted on this site on 06/15/2005, reviewed on 08/12/2010, and revised by the state on 06/23/2006.

Hawaii began submitting to NEMSIS in the third quarter of 2008. They are collecting 100% of the NEMSIS data set. 100% of their agencies are submitting 100% of their EMS runs.

The Hawaii Department of Health, Division of EMS (HDOHEMS) data committee consisting of representatives from the HDOHEMS, the trauma registry, and field providers created the HEMSIS dataset in 2005. Originally consisting of 109-elements the HEMSIS dataset provided data collection to the HDOHEMS for 2 years. The HDOHEMS early on was committed to participation in NEMSIS and developed their system prior to the final release of NEMSIS. There was no way the HDOHEMS could fully anticipate the final NEMSIS process as it matured
over the past years. This version 2 of the HEMSIS data dictionary incorporates the NEMSIS national data elements along with some of the original data elements that the HDOHEMS requires to provide adequate monitoring and reporting of Emergency Medical Services (EMS) activity in the State of Hawaii.
