This version of the State EMS Forms was posted on this site on 7/31/2007, and revised by the state on 7/26/2009. Last updated 7/29/2009. Illinois Department of Transportation Division of Traffic Safety Traffic Records Coordinating Committee states in their June 5th, 2006 Strategic Plan for Traffic Records Improvement: EMS transport providers are required to submit all pre-hospital care reports to the state data repository. We do not maintain data from EMS agencies licensed as non-transport providers. About 40 percent of the reports are submitted in a scannable paper format (bubble form) and the remaining data are collected using EMS field collection software. The EMS Division has not adopted the new data dictionary for the National Emergency Medical Services Information System (NEMSIS). The 2006 NHTSA Traffic Records Assessment reports the following information on data quality: Timeliness EMS providers are required to submit all pre-hospital patient care reports to the state quarterly, but this is not measured. Consistency The state has not adopted the standard NEMSIS data dictionary and data collection protocol. Completeness and Accuracy There is no tracking of EMS providers’ compliance with reporting requirements to the state data repository. No penalties are levied against providers that are not compliant.
