MD - Maryland
This version of the State EMS Forms was posted on this site on 08/27/2007, and revised by the state on 9/27/2005. Last updated 2/13/2009. As of January 2009, per the Executive Director's update: EMAIS®. The enhanced, printable Patient Care Report (PCR) from EMAIS® has been tested and deployed. This final version of the report incorporates recommendations from both EMS jurisdictions and hospitals that use EMAIS®. Also, and e-commerce style password reset feature has been launched to assist providers with password problems. Currently, eighteen jurisdictions are utilizing EMAIS®, along with seven Operational Programs statewide. Targeted Data Exports have been constructed for EMAIS® Jurisdictions and Programs to use in local EMS management. Grant funding has been secured for the analysis of the Maryland EMS Data Collection System, and recommendations for the future eMAIS 3.0. Each emergency services unit responding to an incident involving the delivery of emergency care is required to complete a run report. Most EMS Operational Programs meet this requirement either by using the state-provided paper Maryland Ambulance Information System (MAIS) runsheet or by participating in the state’s electronic data collection system (EMAIS). Several EMS Operational Programs complete a self-developed form, but must submit their data in a specified format. We presently have 16 of our 24 jurisdictions submitting data through a self developed, web based application known as EMAIS. We strive for complete statewide electronic submission in the future. Per the 2007-2008 MIEMSS Annual Report: This year’s third funding request for EMAIS® assessment for National EMS Information System (NEMSIS) compliance was granted for FY 2009. In accordance with both the State EMS Plan’s and the State Highway Safety Plan’s prioritized objectives, EMS data are essential in the improvement of prevention and ultimate patient outcomes of victims of motor vehicle crashes. Incorporation of prehospital data requirements from the NEMSIS effort has been a key to our data standardization effort this year. Click on the thumbnails below to view a full sized image.
