This version of the State EMS Forms was posted on this site on 08/27/2007, and revised by the state on 9/27/2005. Last updated 10/29/2008. Each emergency services unit responding to an incident involving the delivery of emergency care is required to complete a run report. Most EMS Operational Programs meet this requirement either by using the state-provided paper Maryland Ambulance Information System (MAIS) runsheet or by participating in the state’s electronic data collection system (EMAIS).
