This version of the State EMS Forms was posted on this site on 7/9/2010, and revised by the state on 10/15/2005 . New Hampshire has been submitting to NEMSIS since January 2006. There are 83.6% of the states agencies submitting 100% of their data. The state is submitting 93% of the National EMS Dataset. New Hampshire has implemented their
web-based, statewide data system called TEMSIS (Trauma Emergency Medical
Services Information System). It gives New Hampshire's 279 agencies the
flexibility to collect their own data and then upload to the state via XML or
