This version of the State EMS Forms was posted on this site on 7/8/2010, and revised by the state on 10/29/2005. 

New Jersey began submitting to NEMSIS in 2009.

Currently there are two electronic data collection initiatives being undertaken that involve the paid and volunteer EMS providers.  OEMS has adopted EmsCharts as the state paid pre-hospital provider data collection platform.  EmsCharts has been approved as a National Emergency Medical Services Information System (NEMSIS) version 2.2.1 Gold Standard compliant. This is a web-based application that provides near real time data submission with inherent edit and logic checks. OEMS has set a 2009 deadline for all paid services to complete the migration from their existing data collection software application to the EmsCharts software.  EmsCharts will be the sole paid EMS provider data collection and submission software product that will be accepted at the OEMS data repository. Currently there are approximately 170 EMS providers using this software. There are 700 providers in the state.

Volunteer rescue squads are not required to complete ambulance reports.

ALS and licensed BLS services that are required to complete PCRs only submit aggregate data to the NJOEMS.

 

Per State the State of New Jersey in an announcement Dated September 30th 2008:

On-Line Electronic Patient Care Reporting is now available to all EMS agencies (.PDF 362k)

Per administrative code the state collects aggregate data quarterly.
