This version of the State EMS Forms was posted on this site on 11/05/2008, and revised by the state on 10/29/2005.

Per State the State of New Jersey in an announcement Dated September 30th 2008:

On-Line Electronic Patient Care Reporting is now available to all EMS agencies (.PDF 362k)

Volunteer rescue squads are not required to complete ambulance reports.

ALS and licensed BLS services that are required to complete PCRs only submit aggregate data to the NJOEMS.

Per administrative code the state collects aggregate data quarterly.
