This version of the State EMS Forms was posted on this site on 08/26/2010, and revised by the state on 12/2/2005.

Wisconsin has a newly implemented, web-based, statewide data system called WARDS (Wisconsin Ambulance Run Data System). It gives Wisconsin's 458 agencies the flexibility to collect their own data and then upload to the state via XML or enter their runs via a web browser using an online run form. The WARDS system was developed for Wisconsin by ImageTrend, Inc.
